Description
Paclitaxel is a taxane chemotherapy agent derived from the Pacific yew tree that functions as a microtubule-stabilizing drug used in various solid tumors including colorectal cancer. In CRC, paclitaxel is primarily utilized in metastatic settings as part of combination regimens or in refractory disease when standard fluoropyrimidine-based therapies have failed. It is often combined with other agents like carboplatin or targeted therapies in clinical trial settings for advanced colorectal malignancies. The drug may be considered across different molecular subtypes of CRC, including KRAS, NRAS, and BRAF mutated tumors.
Mechanism of Action
Paclitaxel binds to the ?-tubulin subunit of microtubules and promotes their assembly while preventing depolymerization, effectively stabilizing the microtubule structure. This disruption of normal microtubule dynamics prevents proper chromosome segregation during mitosis, leading to mitotic arrest and subsequent apoptosis in rapidly dividing cancer cells. The drug's mechanism is cell cycle-specific, primarily affecting cells in the G2/M phase.
