Description

Melphalan is an alkylating chemotherapy agent used in colorectal cancer primarily through isolated hepatic perfusion (IHP) for patients with liver-confined metastases. This specialized delivery technique allows high concentrations of melphalan to be delivered directly to the liver while minimizing systemic exposure. The approach is reserved for patients with unresectable colorectal liver metastases when standard systemic therapies have been exhausted or are not suitable.

Mechanism of Action

Melphalan functions as a bifunctional alkylating agent that forms covalent bonds with DNA, creating interstrand and intrastrand cross-links that prevent DNA replication and transcription. This mechanism leads to cell cycle arrest and apoptosis, particularly affecting rapidly dividing cancer cells.
