Smoothened (SMO)

Description

Vismodegib is an oral hedgehog pathway inhibitor that targets the Smoothened (SMO) protein, primarily FDA-approved for advanced basal cell carcinoma. In colorectal cancer, vismodegib has been evaluated in combination with standard chemotherapy and bevacizumab for metastatic disease, as well as in tumor-agnostic precision medicine approaches for patients with hedgehog pathway alterations. The drug represents a targeted approach for CRC patients whose tumors demonstrate aberrant hedgehog signaling pathway activation.

Mechanism of Action

Vismodegib selectively binds to and inhibits the Smoothened (SMO) protein, a key transmembrane component of the hedgehog signaling pathway. This inhibition blocks downstream signaling cascade that normally promotes cell proliferation, differentiation, and tissue patterning, ultimately leading to reduced tumor cell growth and survival in cancers dependent on hedgehog pathway activation.
