PDE5 (phosphodiesterase-5)

Description

Udenafil is a phosphodiesterase-5 (PDE5) inhibitor primarily used for erectile dysfunction that is being repurposed in colorectal cancer care for penile rehabilitation following surgical resection. In the context of CRC treatment, udenafil is specifically employed to address erectile dysfunction that commonly occurs after total mesorectal excision and radical resection procedures for rectal and sigmoid colon cancers. This medication serves as a supportive care intervention to help restore sexual function in male patients who have undergone these extensive pelvic surgeries.

Mechanism of Action

Udenafil works by selectively inhibiting phosphodiesterase-5, an enzyme that degrades cyclic guanosine monophosphate (cGMP) in penile smooth muscle cells. By blocking PDE5, udenafil increases cGMP levels, leading to smooth muscle relaxation, vasodilation, and improved blood flow to penile tissues. This mechanism helps counteract the vascular and neurological damage that can occur during radical colorectal surgery procedures.
