Description

Pegfilgrastim is a long-acting pegylated granulocyte colony-stimulating factor (G-CSF) used as supportive care in colorectal cancer treatment. It prevents chemotherapy-induced neutropenia by stimulating the bone marrow to produce white blood cells, particularly neutrophils. In CRC patients receiving myelosuppressive chemotherapy regimens, pegfilgrastim reduces the risk of febrile neutropenia and allows for maintenance of chemotherapy dose intensity.

Mechanism of Action

Pegfilgrastim binds to and activates G-CSF receptors on neutrophil precursor cells in the bone marrow, stimulating their proliferation, differentiation, and maturation into functional neutrophils. The pegylation increases the drug's half-life by reducing renal clearance, while neutrophil-mediated clearance provides self-regulating pharmacokinetics that decrease as neutrophil counts recover.
