Description

Neomycin is an aminoglycoside antibiotic used as supportive care in colorectal cancer treatment, primarily for bowel decontamination before surgical procedures. It is administered orally as part of mechanical bowel preparation protocols to reduce intestinal bacterial load and decrease the risk of surgical site infections following colorectal surgery. Neomycin may also be used in research settings to modify gut microbiota composition prior to interventions like fecal microbiota transplantation in patients with microsatellite instability-high (MSI-H) metastatic colorectal cancer.

Mechanism of Action

Neomycin binds irreversibly to the 30S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis and causing bacterial cell death. When administered orally, it acts locally within the gastrointestinal tract with minimal systemic absorption, selectively reducing gram-negative aerobic bacteria in the colon.
