Description

Carmustine is an alkylating chemotherapy agent from the nitrosourea class that has been evaluated in combination regimens for metastatic colorectal cancer. This lipophilic agent crosses the blood-brain barrier and acts as a DNA cross-linking agent, causing cell cycle arrest and apoptosis in rapidly dividing cancer cells. In colorectal cancer, carmustine has been incorporated into multi-drug combination protocols, though it is not considered a standard first-line therapy for this indication.

Mechanism of Action

Carmustine functions as a bifunctional alkylating agent that forms covalent bonds with DNA, creating interstrand and intrastrand cross-links that prevent DNA replication and transcription. The drug also carbamoylates proteins, particularly enzymes involved in DNA repair, which enhances its cytotoxic effects and prevents cellular recovery from DNA damage.
