Description
Balstilimab is a monoclonal antibody immunotherapy agent that targets the PD-L1 (programmed death-ligand 1) protein on cancer cells and immune cells. In colorectal cancer treatment, balstilimab is used in combination with botensilimab, particularly in patients with microsatellite instability-high (MSI-H) tumors and those with circulating tumor DNA positivity following surgery and chemotherapy. The drug has been evaluated across various colorectal cancer settings, including localized rectal cancer in combination with standard chemotherapy regimens like FOLFOX.
Mechanism of Action
Balstilimab works by binding to PD-L1, blocking the interaction between PD-L1 and its receptor PD-1 on T-cells. This blockade prevents cancer cells from evading immune system detection and allows activated T-cells to maintain their anti-tumor immune response, leading to enhanced tumor cell destruction.
