Description
Vorinostat is a histone deacetylase (HDAC) inhibitor that has been evaluated in colorectal cancer as combination therapy with other agents including immunotherapy and hydroxychloroquine. This oral medication was originally approved for cutaneous T-cell lymphoma but has shown potential activity in solid tumors including CRC through its epigenetic modulation properties. In colorectal cancer, vorinostat is primarily used in combination regimens rather than as monotherapy, with trials comparing vorinostat plus hydroxychloroquine against standard treatments like regorafenib.
Mechanism of Action
Vorinostat inhibits histone deacetylases (HDACs), enzymes that remove acetyl groups from histone proteins, leading to chromatin condensation and gene silencing. By blocking HDAC activity, vorinostat promotes histone acetylation, resulting in chromatin relaxation and reactivation of tumor suppressor genes that are silenced in cancer cells. This epigenetic mechanism can enhance the effectiveness of other therapies and potentially overcome resistance mechanisms in colorectal cancer.
