Description

Plerixafor is a CXCR4 chemokine receptor antagonist that is being explored for its potential to enhance immune cell infiltration into colorectal cancer tumors. While FDA-approved for stem cell mobilization in hematologic malignancies, its use in colorectal cancer represents an off-label application based on its ability to disrupt the tumor microenvironment. In CRC, plerixafor aims to improve immune surveillance by blocking CXCR4-mediated retention of immune cells in the bone marrow and facilitating their migration to tumor sites.

Mechanism of Action

Plerixafor selectively and reversibly binds to the CXCR4 chemokine receptor, blocking its interaction with stromal cell-derived factor-1 (SDF-1/CXCL12). This disruption mobilizes hematopoietic stem cells and immune effector cells from the bone marrow into peripheral circulation. In the context of colorectal cancer, this mechanism may help overcome immune exclusion by promoting T-cell infiltration into the immunosuppressive tumor microenvironment.
