Description
MM-398 (nanoliposomal irinotecan, also known as nal-IRI) is a liposomal formulation of the topoisomerase I inhibitor irinotecan used as chemotherapy for metastatic pancreatic adenocarcinoma and colorectal cancer. The nanoliposomal encapsulation allows for improved drug delivery and altered pharmacokinetics compared to conventional irinotecan. In colorectal cancer, it is typically used in combination regimens for patients with advanced or metastatic disease who have progressed on prior therapies.
Mechanism of Action
MM-398 encapsulates irinotecan within liposomes that circulate longer in the bloodstream and preferentially accumulate in tumor tissue through enhanced permeability and retention effects. Once released, irinotecan is converted to its active metabolite SN-38, which inhibits topoisomerase I, an enzyme essential for DNA replication and transcription, leading to DNA damage and cancer cell death.
