Description
Chidamide is a selective histone deacetylase (HDAC) inhibitor that belongs to the benzamide class of epigenetic modulators. In colorectal cancer treatment, it is being evaluated in combination regimens for microsatellite stable (MSS) and mismatch repair proficient (pMMR) tumors, which are typically less responsive to immunotherapy alone. The drug is used in combination with other agents including immunotherapies, targeted therapies like fruquintinib and regorafenib, and in neoadjuvant chemoimmunotherapy protocols for locally advanced disease.
Mechanism of Action
Chidamide selectively inhibits class I histone deacetylases (HDAC1, HDAC2, HDAC3) and class IIb HDAC10, leading to increased acetylation of histones and non-histone proteins. This epigenetic modulation results in reactivation of tumor suppressor genes, enhanced antigen presentation, and increased tumor immunogenicity, potentially making MSS colorectal tumors more susceptible to immunotherapy approaches.
