Description
Avelumab is a fully human monoclonal antibody immunotherapy that blocks the PD-L1 (programmed death-ligand 1) pathway to enhance anti-tumor immune responses. In colorectal cancer treatment, avelumab is used in combination with other agents such as cetuximab for rechallenge strategies in RAS wild-type metastatic disease and with 5-FU based chemotherapy for adjuvant treatment of stage 3 MSI-high or POLE mutant colon cancer. The drug has shown particular promise in microsatellite instability-high (MSI-H) colorectal cancers, which are more responsive to immune checkpoint inhibition.
Mechanism of Action
Avelumab binds specifically to PD-L1 and blocks its interaction with PD-1 receptors on T-cells, preventing the tumor from suppressing immune system activation. By inhibiting this immune checkpoint pathway, avelumab allows cytotoxic T-lymphocytes to maintain their anti-tumor activity and promotes sustained immune-mediated tumor cell destruction.
