Description

Atezolizumab is a monoclonal antibody immunotherapy agent that blocks the PD-L1 checkpoint pathway to enhance immune system recognition of cancer cells. In colorectal cancer, it is primarily being utilized in combination regimens for metastatic disease, particularly in microsatellite stable (MSS) tumors when combined with chemotherapy and bevacizumab, and in patients with high immune cell infiltration. The drug is also being explored in combination with stereotactic body radiation therapy for MSS metastatic colorecral cancer and in patients with circulating tumor DNA-positive gastrointestinal cancers.

Mechanism of Action

Atezolizumab binds to programmed death-ligand 1 (PD-L1) expressed on tumor cells and tumor-infiltrating immune cells, preventing interaction with PD-1 and B7.1 receptors on T-cells. This blockade removes the inhibitory signals that normally suppress T-cell activation, allowing cytotoxic T-lymphocytes to recognize and attack cancer cells more effectively.
